You can use this page to print your permission slips and 2008 health forms for upcoming Ground Zero events. Just click the link below for the event you want to attend and a new window will open with your permission slip inside. Just print the contents of that window and have your parent or guardian fill it out. If you have not turned in a health form for 2008, then you will need one of those too. You only have to do one health form a year unless your medical conditions or emergency contacts change. Make sure you have yours completed and turned in so that you can attend all our events. Bring your completed 2008 health form and permission slip, with your money, to GZ. It's that simple.
